Reabilitação pélvica após cirurgias oncológicas e efeitos na continência função sexual e qualidade de vida: uma revisão de literatura

Introdução: As cirurgias oncológicas pélvicas, incluindo a prostatectomia radical, as ressecções colorretais com preservação esfincteriana e as intervenções ginecológicas, frequentemente comprometem a continência urinária e fecal, a função sexual e a qualidade de vida. A reabilitação pélvica conduzida por fisioterapeutas é proposta como estratégia conservadora de primeira linha para essas sequelas. Objetivo: Descreveu-se o impacto da reabilitação pélvica na continência, na função sexual e na qualidade de vida após cirurgias oncológicas pélvicas. Métodos: Revisão de literatura, de abordagem narrativa e qualitativa. A busca foi realizada de janeiro a junho de 2026 nas bases Medical Literature Analysis and Retrieval System Online (MEDLINE), Excerpta Medica Database (Embase), Scientific Electronic Library Online (SciELO), Literatura Latino-Americana e do Caribe em Ciências da Saúde (LILACS), Physiotherapy Evidence Database (PEDro) e Cochrane Library, com termos controlados dos Descritores em Ciências da Saúde (DeCS) e do vocabulário Medical Subject Headings (MeSH) combinados por operadores booleanos. Foram considerados elegíveis ensaios clínicos, revisões, diretrizes e documentos oficiais em português, inglês e espanhol, selecionados por leitura de títulos e resumos e, em seguida, dos textos completos, com análise descritiva por eixos temáticos. Resultados: Das 32 referências utilizadas, 23 compuseram o corpus analítico, publicadas entre 2007 e 2026, com predomínio de ensaios clínicos randomizados (39,1%) e revisões sistemáticas sem metanálise (30,4%). O treinamento dos músculos do assoalho pélvico foi a intervenção mais descrita e associou-se ao retorno mais precoce da continência urinária após prostatectomia. Na síndrome da ressecção anterior baixa, os benefícios relatados restringiram-se a domínios sintomáticos específicos. No câncer ginecológico, intervenções multimodais associaram-se à melhora da dispareunia e da função sexual. Conclusão: A reabilitação pélvica mostrou-se abordagem conservadora aplicável aos três contextos cirúrgicos, com evidência mais consistente para a continência urinária precoce e resultados heterogêneos nos demais desfechos.
