Disfagia pós-extubação e reabilitação fonoaudiológica e motora no paciente crítico: Uma revisão narrativa

Resumen

Introdução: A disfagia pós-extubação é frequente em pacientes críticos e pode retardar a ingestão oral, favorecer aspiração e agravar desfechos. Objetivo: Analisar ocorrência, fatores associados, métodos de reconhecimento e evidências sobre reabilitação fonoaudiológica e motora da disfagia pós-extubação em adultos criticamente enfermos. Métodos: Realizou-se revisão narrativa com buscas em agosto de 2026 no PubMed, na Scientific Electronic Library Online e na Biblioteca Cochrane, complementadas por rastreamento de referências. Foram priorizados estudos de 2021 a 2026 sobre adultos submetidos à intubação orotraqueal, reconhecimento precoce e reabilitação. Vinte e três publicações compuseram a síntese, com apreciação dos desenhos conforme as perguntas que podiam responder. Resultados: A estimativa agrupada mais recente foi de 35% (IC 95%: 25–46%), enquanto uma síntese anterior encontrou 41%; diferenças de população, momento e método impedem comparação direta. Aspiração silenciosa e acurácia incompleta do exame à beira-leito apoiaram rastreio estruturado, avaliação fonoaudiológica e investigação instrumental seletiva. Intervenções fonoaudiológicas apresentaram resultados favoráveis para ingestão oral e pneumonia, mas os protocolos foram heterogêneos e as amostras, pequenas. A reabilitação motora favoreceu condições sistêmicas para alimentação, embora a evidência de efeito direto sobre a deglutição tenha permanecido escassa. Conclusão: A literatura sintetizada indica que prontidão clínica, rastreio, avaliação especializada e reabilitação interdisciplinar podem orientar a progressão segura da via oral. A duração da intubação aumenta a suspeita, mas não confirma disfagia; o cuidado requer reavaliação seriada. A evidência terapêutica permanece limitada e heterogênea.
