Mobilização precoce após cirurgia do intestino: Função intestinal e alta hospitalar: Revisão narrativa

Resumen

Introdução: A recuperação após ressecção intestinal eletiva é acompanhada por três marcos: a mobilização precoce, o retorno da função intestinal e a alta hospitalar. Objetivo: Avaliou-se criticamente a suposta cadeia causal entre esses marcos após cirurgia do intestino, delimitando a contribuição possível da fisioterapia. Métodos: Revisão narrativa da literatura, de caráter descritivo, crítico e qualitativo, com busca estruturada. A busca foi realizada entre junho e agosto de 2026 na Medical Literature Analysis and Retrieval System Online, acessada por meio do PubMed, na Scientific Electronic Library Online e na Literatura Latino-Americana e do Caribe em Ciências da Saúde, com rastreamento de listas de referências e consulta a documentos oficiais. As revisões Cochrane foram recuperadas nessa primeira base, sem busca direta na Cochrane Library. Priorizaram-se publicações de 2021 a 2026, em português, inglês e espanhol. A seleção foi consensual, em leitura conjunta dos autores, sem triagem em duplicata independente. Resultados: Foram analisadas 53 fontes. A mobilização precoce é recomendação consensual, mas a maioria dos pacientes não atinge as metas propostas, e os ensaios que ampliaram o tempo fora do leito não modificaram capacidade de marcha, função pulmonar, complicações nem tempo de internação. Exercício supervisionado com dose definida e preparação pré-operatória mostraram efeitos mais consistentes, e parte do atraso da alta decorre de fatores organizacionais. Conclusão: A contribuição da fisioterapia parece residir menos no cumprimento de metas horárias e mais na preparação pré-operatória, na dose terapêutica do exercício e na remoção das barreiras à verticalização em pacientes com reserva funcional reduzida.
