Cansaço na quimioterapia: exercício físico, força muscular e continuidade do tratamento: Uma revisão narrativa

Resumo

Introdução: A fadiga relacionada ao câncer pode intensificar-se durante a quimioterapia, comprometer a força e dificultar atividades necessárias à continuidade do cuidado. Objetivo: Analisar os efeitos do exercício físico sobre fadiga e força muscular durante a quimioterapia e discutir sua relação com dose, atrasos e conclusão do tratamento. Métodos: Revisão narrativa com busca estruturada na Medical Literature Analysis and Retrieval System Online via PubMed, Scientific Electronic Library Online, Literatura Latino-Americana e do Caribe em Ciências da Saúde via Biblioteca Virtual em Saúde, Physiotherapy Evidence Database e Cochrane Library, realizada entre julho e setembro de 2026. Foram priorizados estudos de 2021 a 2026, sem excluir trabalhos seminais. A seleção considerou títulos, resumos e textos integrais disponíveis. Resultados: Sínteses recentes indicam redução pequena da fadiga com exercícios aeróbicos ou resistidos em curto prazo. O treino resistido preserva sobretudo a força de membros inferiores, embora os resultados variem conforme modalidade, supervisão e população. Alguns ensaios apontam menos ajustes ou maior conclusão da quimioterapia, mas outros não mostram diferença na intensidade relativa de dose. A evidência direta para continuidade permanece escassa e heterogênea. Conclusão: O exercício individualizado deve integrar o manejo fisioterapêutico da fadiga e da perda de força durante a quimioterapia. Sua indicação pode apoiar a tolerância funcional, mas não garante a manutenção do esquema antineoplásico.
