Abstract

Introduction: Scheuermann's disease is a sagittal plane deformity characterized by structural thoracic hyperkyphosis with the potential to progress during growth. Although conservative treatment includes scoliosis-specific physiotherapy and bracing, studies describing the effects of this combined approach remain scarce, particularly regarding bone remodeling and the maintenance of treatment outcomes over time. Objective: To report the long-term effects of combining the Scientific Exercises Approach to Scoliosis (SEAS) with rigid bracing in the conservative treatment of Scheuermann's disease. Methods: This retrospective case report describes a 14-year-old male patient with a 74° thoracic hyperkyphosis, previous surgical indication, and skeletal immaturity. Treatment consisted of a rigid brace worn initially for 23 hours per day, with progressive weaning, combined with scoliosis-specific physiotherapy and home exercises for 24 months, followed by a 12-month follow-up period without intervention. Results: Clinical and radiographic improvements were observed, including a reduction in thoracic kyphosis, improved global sagittal alignment, and bone remodeling of the wedged vertebrae, as demonstrated by changes in the ALFA angle. The results remained stable after one year of follow-up without active treatment. Conclusion: The combination of scoliosis-specific physiotherapy and rigid bracing proved to be an effective alternative to surgical treatment in adolescents with Scheuermann's disease and skeletal immaturity, promoting structural correction and maintaining long-term treatment outcomes.
