Introduction: Fibromyalgia is a chronic condition characterized primarily by widespread musculoskeletal pain, which may be associated with functional impairment, reduced physical activity, and a diminished quality of life. In this context, therapeutic exercise stands out as a key non-pharmacological strategy, with various modalities employed in the rehabilitation of these patients. Objective: To synthesize available evidence regarding the effects of therapeutic exercise on pain, functionality, and quality of life in patients with fibromyalgia. Methods: This is an integrative literature review of a qualitative and descriptive nature, conducted using the PubMed/MEDLINE, SciELO, and BVS databases. A total of 77 records were identified, 17 of which were duplicates. Following screening and eligibility assessment, 10 studies comprised the final sample. Different study designs and exercise modalities were analyzed, focusing primarily on pain, functionality, and quality of life. Results: The studies demonstrated favorable effects of aerobic, resistance, and functional exercises, as well as rehabilitation programs, particularly regarding pain reduction and improvements in physical function and quality of life. Benefits regarding muscle strength, balance, cardiorespiratory fitness, well-being, and depressive symptoms were also observed. However, the heterogeneity of protocols, interventions, and follow-up periods made it difficult to identify a single exercise modality superior to the others, reinforcing the need to individualize therapeutic prescriptions. Conclusion: Therapeutic exercise is a relevant strategy in the non-pharmacological management of fibromyalgia, potentially contributing to pain reduction and improvements in functionality and quality of life. Nevertheless, more standardized studies are needed to determine the most appropriate exercise parameters and the long-term maintenance of their effects.
