Exercício terapêutico, decisão clínica e transição para artroplastia na osteoartrite do joelho: Uma revisão narrativa

Resumo

Introdução: O exercício terapêutico e a artroplastia integram o cuidado de adultos com osteoartrite sintomática do joelho, mas as evidências sobre exercício abrangem populações mais amplas, enquanto comparações diretas com cirurgia envolvem pacientes selecionados e elegíveis ao procedimento. Objetivo: Analisar benefícios, limites e critérios que orientam a transição do tratamento conservador para a cirurgia. Métodos: Revisão narrativa com busca no PubMed, atualizada em 18 de setembro de 2026, consulta a diretrizes institucionais e rastreamento bibliográfico. Priorizaram-se publicações de 2021 a 2026, preservando ensaios de referência e seguimentos anteriores. A síntese distinguiu comparações diretas, estudos observacionais, revisões e diretrizes. Resultados: O exercício produziu melhora de dor e função, com magnitude variável. Em pacientes elegíveis para cirurgia, a artroplastia associada ao tratamento conservador proporcionou maior benefício que o tratamento conservador isolado, com mais eventos adversos graves. Em estudos sem grupo não exposto ao exercício, parte dos pacientes optou por adiar a cirurgia; o achado não estabelece causalidade nem comprova recuperação funcional. Conclusão: Programas multimodais impedem atribuir seus efeitos exclusivamente ao exercício. A indicação estabelecida não deve ser prolongada automaticamente por novos ciclos de fisioterapia, e a reabilitação permanece pertinente quando se escolhe operar. O exercício constitui tratamento fundamental; a artroplastia é opção para limitação persistente relevante apesar de cuidado conservador adequado ou quando este é inadequado. A escolha requer reavaliação funcional, delimitação da aplicabilidade das evidências e decisão compartilhada.
