Efeitos da reabilitação após cirurgia bariátrica sobre massa magra e capacidade funcional: Uma revisão narrativa

Resumo

Introdução: A perda ponderal acelerada após cirurgia bariátrica melhora mobilidade e saúde metabólica, porém inclui reduções de massa corporal magra, massa livre de gordura e massa muscular esquelética, sobretudo nos primeiros meses. Essa trajetória pode coexistir com queda da força absoluta, mesmo quando testes funcionais melhoram pelo menor peso corporal. Objetivo: Analisar os efeitos da reabilitação física após cirurgia bariátrica sobre composição corporal, força muscular, capacidade cardiorrespiratória e desempenho funcional. Métodos: Foi realizada revisão narrativa crítica com buscas no PubMed/MEDLINE e na Cochrane Library, atualizadas pela última vez em 18 de setembro de 2026. Foram priorizados ensaios clínicos, revisões sistemáticas, metanálises e diretrizes sobre exercício pós-operatório, composição corporal, força e capacidade cardiorrespiratória; o corpus final reuniu 23 publicações. Resultados: O treinamento físico apresentou efeitos mais consistentes sobre força, consumo de oxigênio e testes de desempenho do que sobre a preservação das medidas de tecido magro. Programas multicomponentes ou com resistência progressiva, maior frequência semanal, supervisão e continuidade apresentaram os sinais mais favoráveis, mas resultados heterogêneos impedem definir uma dose universal. Intervenções de baixa exposição e suplementação proteica isolada não asseguraram preservação muscular. Conclusão: A recuperação funcional apresentou resultados mais consistentes após cirurgia bariátrica, enquanto a preservação das medidas de tecido magro permaneceu variável e dependente de dose, adesão e contexto clínico. Protocolos individualizados e prolongados são mais defensáveis do que prescrições uniformes.
