Characteristics of patients with postural hypotension by tilt test and nursing assistance

Introduction: Postural hypotension (PH) may not be diagnosed during the consultation, due to it being late (> 3 min), requiring the tilt test (TT). IT is carried out by a team, with a qualified cardiologist and nursing staff, in a suitable room. Objective: To describe the clinical profile of patients with pre-syncope or syncope and with PH at TT, the time of diagnosis, symptoms and the role of nursing during this test. Methods: Prospective and observational study with 110 patients who underwent TT and had a PH response. TT was performed at 70 degrees for a period of up to 30 minutes, in an equipped room with a cardiologist and nursing team. Results: The mean age of the patients was 63.7 ± 17.2 years, 50% (n=55) female, 47.2% (n=52) with trauma related to PH. The most frequent comorbidities were systemic arterial hypertension (n=69), ischemic heart disease (n=17), diabetes (n=18) and chagasic disease (n=11). The average time for PH to occur was 19.8 min, and 42.7% (n=47) of patients had symptoms. The mean absolute drop in blood pressure compared to the lying position was 28.6/6.5 mmHg and there was an increase of 9.7 bpm in heart rate. There was a need for infusion of saline and atropine during TT. Conclusion: The patient profile demonstrated significant morbidity. The TT diagnosis time was almost 20 min. Nursing assistance was necessary for the preparation of patients and for interventions.
