Medida indireta da pressão arterial em membros inferiores: revisão integrativa

Resumo

Introdução: técnica auscultatória realizada nos membros superiores, feita na posição sentada, é a mais utilizada. Entretanto, é possível mensurar também nos membros inferiores e nas posições em pé ou deitada. Objetivo: analisar as evidências científicas disponíveis na literatura sobre a medida indireta da arterial nos membros inferiores. Métodos: revisão integrativa, com estratégia de busca utilizando as bases Academic Search Premier, CINAHL, Embase, LILACS, PubMed, Scopus e Web ofScience, com os termos Ankle, Arterial Pressure, Blood Pressure, Blood Pressure Determination, Leg, Lower Extremity, Thighe e seus sinônimos. Adicionada busca manual nas referências dos estudos incluídos. Resultados: a amostra final foi constituída por 23 estudos primários, os quais foram divididos em três categorias temáticas. A primeira refere-se à comparação de valores de pressão arterial; a segunda à comparação de métodos de medida da pressão arterial; e a terceira a recomendações. Assim, nesses verificou-se que os valores da pressão arterial nos membros inferiores possuem uma diferença média de 11,2 a 36,5 milímetros de mercúrio quando comparados ao braço. Além disso, a técnica realizada nos membros inferiores causa desconforto e dor. Conclusão: o tornozelo e a coxa são alternativas viáveis em casos em que a medida indireta da pressão arterial não pode ser realizada no braço. Entretanto, a opção supracitada não deve ser utilizada como substituta ao braço, devido a discrepância dos valores encontrados e sua importância na prática clínica.
